SHS Admission Form

Requirements for Incoming Senior High School (SHS) Students

Please prepare and submit the following:

  • 1st to 2nd Grading Card (if available)
  • Two (2) photocopies of PSA Birth Certificate
  • Four (4) pieces 2×2 ID photos (with name tag and white background)
  • One (1) Long Brown Envelope
  • One (1) White Long Folder

Please prepare and submit the following:

  • Certificate of Transfer (Original and Photocopy)
  • Certificate of Grades / Report Card (Original and Photocopy)
  • Certificate of Good Moral Character (Original and Photocopy)
  • Two (2) photocopies of Birth Certificate
  • Four (4) pieces 2×2 ID photos (with name tag and white background)
  • One (1) Long Brown Envelope
