- Photocopy of PSA Birth Certificate – 2 pcs
- 2×2 ID Photos – 4 pcs, white background with name tag
- White Long Folder – 1 pc
- Brown Long Envelope – 1 pc
Requirements for Transferee Students
Please prepare and submit the following:
- Certificate of Transfer (Original and Photocopy)
- Certificate of Grades / Report Card (Original and Photocopy)
- Certificate of Good Moral Character (Original and Photocopy)
- Transcript of Records – Original & Photocopy
- Photocopy of PSA Birth Certificate – 2 pcs
